Milivoje Novakovic joined German outfit FC Koeln from Bulgarian PFC club Litex Lovech, where he rose to prominence. Having begun his career with Olimpija Ljubljana he went on to play for Austrian club LASK Linz.
With Litex he reached the UEFA Cup 2005-06 round of 32, came third in the Bulgarian A Professional Football Group and became the championship's top goalscorer.
He had been linked with German team FC Koeln, Israeli team Beitar Jerusalem FC and Bulgarian champions Levski Sofia, but he nevertheless played for Litex in the UEFA Cup 2006-07 against his compatriots FC Koper and AC Omonia from Cyprus despite his wish to continue his career in a different team.
He finally joined FC Koeln in late August 2006 for around €1.5 million. He was the top goalscorer in the 2. German Bundesliga (second division) during the 2007-08 season with the club.
He was appointed captain of the first team squad for FC Koeln in September 2008. He scored 16 goals in the German Bundesliga in the 2008-09 season.
He is one of Slovenia's main strikers and has even scored a hat trick against Trinidad & Tobago.

In November 2009 he was stripped off his captaincy due to a dispute with FC Koeln's new manager Zvonimir Soldo.
